For #2 I read a story by [livejournal.com profile] nong_pradu called Not By Your Hand (https://www.fanfiction.net/s/8008226/1/Not-By-Your-Hand) that fits everything you're looking for except that there is permanent damage. Still, it's worth a read. Nong has a gorgeous way with words. I don't think you'd be sorry you read it.
