It's a self-rec if you don't mind. But it's almost exactly what you're looking for in #1 J2 here, so... Free Falling 'verse (http://evening-spirit.livejournal.com/17126.html) There are various stories but the last two (chronologically) deal with emerging DID. Now that I think of it, it's not DID in full blossom, so maybe not what you're looking for, but maybe it'll interest you anyway. I think you should start with Dissociation (http://evening-spirit.livejournal.com/6146.html#cutid1) and see if it's your thing.
