One of my favorites is MRSA (http://www.fanfiction.net/s/3160168/1/MRSA) by Janessa11. This is on FF.net but I thought I read it on LJ.

Also, there was an excellent, and terribly sad, Mystery Spot fic where Dean's death by taco was beautifully, painfully and heartbreakingly portrayed. I did a search, but couldn't find it. Maybe somebody else will know it?

PS, I agree with tifaching, wave_obscura is one of LJ's mad demons of Dean's ManPain and her Houdini Series is top notch.
