Maybe This is It?
The only fic I know where Cas is transgendered in an A/B/O universe is "Angry Inches" by ghostyouknow. (http://archiveofourown.org/works/772410)

Cas is exactly how you described in your post, he was born Omega but is actually an Alpha in terms of gender.
