Hey :D if you don't mind self recs, my fic clockwork of lungs (http://archiveofourown.org/works/6911356) fulfils both A & B, in a roundabout sort of way.

There are three fics by ameliacareful that concern this sort of stuff- A Failure of Gravity (http://archiveofourown.org/works/5679571""), House Guest (http://archiveofourown.org/works/6109634), & Unreal (http://archiveofourown.org/works/6891589).

Askance's fic body of proof (http://archiveofourown.org/works/4214805/chapters/9527085) is partially concerned with what happened to Sam in the Cage (it's also excellent & very upsetting.) No Wincest in this one, though.
