Implied wincest. Dean doesn't make a deal instead he buries Sam and Sam revives while he's being buried scaring Dean. Seems Sam made a deal with YED to lead his armies and to keep Dean with him. If I remember right Sam kisses Dean and kills him and the fic ends with Sam waiting for Dean to revive Deans death and return is supposed to make him like Sam.
