Have you read the Bright Lights of Disturbia? It's an extremely long Dean/Sam fic, with seriously-fucked-up!boys. There's non-con(Dean/OMC's) and dub-con(Sam/Dean-ish), mental abuse of Dean by YED. Not to mention the fact that it's flawlessly written and is one of my favorite stories ever read.

I suggest "Almost" made me want to wrap Dean in a blanket and give him cuddles and pie.

During Sam's first year at Stanford Dean arrives to leave Sam his car and then checks himself into a mental institution. He won't speak. When something bad happens there Dean finally starts talking to Sam and leaves with him. Sam has to figure out what happened to Dean so he can help him get better, especially if he wants to carry on the relationship Dean seems to want to rekindle with him. Thier father may cause some problems when he gets involved
