I know this fic doesn't exactly match what you are looking for, but it is a great fic. It's called 'Crows in the Wheatfield' by Ontara. It is set pre-series, with Sam at Stanford. Dean and John are on a hunt and Dean gets hurt pretty badly. This is more a Dean h/c fic but Sam does visit Dean and John, though not till way later in the story and only for a couple of chapters. But the time that Sam is with Dean and John is an interesting dynamic. I love this story.
