#2: (None of these are like "Sam is diagnosed with a mental illness!" but they all centre around Dean caring for a mentally ill Sam, to some extent, I think.)

The Holy Trinity (http://checkthemargins.livejournal.com/4991.html) fits sort of, as Sam's OCD is strong enough that I think it would be considered a mental illness.

The Only Thing That's Real (http://runedgirl.livejournal.com/54434.html) is PTSD-ish, with Sam having some pretty violent flashbacks. Definitely hurt!Sam.

Antifreeze (http://obstinatrix.livejournal.com/104850.html) definitely has a mental illness vibe, I'm not sure exactly what you'd call it, probably PTSD again. It's really good, too.

Dictionary for a Dead Language (http://britomart-is.livejournal.com/154955.html) is pretty awesome, and deals with Dean basically nursing a brain-dead Sam back into a working human being.
