There is also Pink Is Goth's "Sandpiper" with Jensen suffering from a serious heart disease - it's still a WIP, but she updates fairly regularly. I love it

I recommend The Sandpiper by pinkisgoth. It is a lengthy fic (28 Ch. so far) where Jensen is seriously ill with a life-threatening disease, but finds hope as his friendship with Jared achieves new heights. It is very well written, that you can clearly imagine the space and places the author boys occupying, physically, mentally, and emotionally. It's also a WIP, so if you aren't someone who reads those, you may want to bookmark it for the future.

The author also has a completed historical fic about the J's meeting and becoming (more than) friends, when so many things in this world were misunderstood. Jensen is sick in this one as well, but as the disclaimer on the heading says, it is NOT a death fic. It’s called After the Frost.

(Sorry, I kinda got a soft spot for Sick!Jensen/Caregiver!Jared *blushes*)
