spn_littlebro is an LJ community dedicated to recs for hurt!Sam (and hurt!Jared). They have a monthly newsletter, and theme posts where anyone can rec a fic (or art, music video, etc.) based on kind of hurt (illness, injury, psychological trauma, etc.). The recs are collected at their delicious archive (http://delicious.com/spn_littlebro) which you can filter further (by word count, more specific types of hurt, etc. For example, this tag (http://delicious.com/spn_littlebro/what%3Dcase.fic) will deliver case fics. You can also filter to specifically get SPN gen... or just check the info found in the tags.)

Thee are also a couple LJ communities I like for whump: ohsam for hurt!Sam and hoodie_time for hurt!Dean. Have you discovered those yet?

Finally, K_Hanna_Khorossy on ff.net (http://www.fanfiction.net/u/946711/K_Hanna_Korossy) delivers tons of h/c, and is equal opportunity Dean!whump and Sam!whump... many of her fics are not casefics, though (i.e. lots of missing scenes, episode tags, etc.), so skim the summaries and word counts to find what you'd like.
