It's Dean/Cas and I found the summary of the story: “Dean is diagnosed with stage three brain cancer and has to do chemotherapy for three months in order to shrink the tumour enough to be cleared for surgery.Cas is absent in that time,which makes Dean wonder if his disease is to blame for that.Minutes after his surgery is done,Cas appears and the two have their last Earth-bound conversation.” if that's at all useful.
