Suddenly I really want to read fics were Dean or Jensen is smoking.
Prefebly with hurt!Dean hurt!Jensen.
J2 is awsome and so are Wincest!

If you don't mind self-rec, I have written two fics where Jensen is smoking.

The first one, misplaced childhood, deals with sexual abuse on children, so maybe it's not an easy read. Jensen smokes. He also has an addiction problem so if you're looking for hurt as well... Anyway, here's the link on a03 : http://archiveofourown.org/works/2051769/chapters/4456944

The aforementioned Drive verse was the first thing that came to my mind.

Also, there's [livejournal.com profile] runedgirl's short J2 fic Cigarette Smoking Can be Hazardous to Your Sexuality (http://runedgirl.livejournal.com/9170.html). There's no hurt!Jensen, but it's perfect, just like everything else by this author.
