Sam out then back in  
Random urge that just grabbed me.

Can anyone recommend a fic where Sam wasn't pulled back in during season 1? Where he stayed out with Jess and told Dean to shove it, completely turning his back on him. Then, for whatever reason, he needs Dean later for help. Preferably something supernatural but I'll take anything. That he then needs to take his family and find his brother and beg him for help.

Would really love if Cas is there with Dean (together or gen).

Have Sam slowly realize all that's happened and all.

This isn't exactly what you're looking for, but it's an amazing little story that packs a hell of a punch in the climax. I don't want to say too much, because I'll spoil it, but it does have a reasonable amount in common with your ask.

Shifter by LadyLini, from this years DeanCas big bang! It's post S10, where Sam never came back and everything happened without him, now he's met up with Dean and is left floundering, wondering just who the heck this Cas guy is and how everyone the Winchesters have ever known is dead.
