patient!dean, doctor!castiel, nurse!sam, AU?  
Hi!

I've been looking for a specific fic.
I remember that Castiel is a doctor at a mental hospital, and he meets patient Dean.
Sam works at the same hospital as a nurse or something.
Dean thinks he and Sam are brothers, but they aren't.

I think I read it at AO3, but I'm not sure.

Can anyone help me?
I'm going out of my mind.
